Change Leadership & Strategy
|
- A clear people change plan for each transformation workstream (scope, audiences, impacts, risks, approach, timeline and measures of success).
- A shared view of what will change for different groups (processes, roles, skills, ways of working), documented and kept up to date as plans evolve.
- Practical advice to programme and project leads so delivery plans include the time, resources and activities needed for adoption.

Stakeholder Engagement & Leadership Support
|
- A stakeholder map and engagement plan that sets out who needs to be involved, how we will listen, and how feedback will shape decisions.
- Sponsor and leader support plans (including briefing packs and talking points) so leaders can explain the change clearly and consistently.
- Regular check-ins with leaders and key groups to surface barriers early and agree actions that build trust, ownership and readiness.

Impact, Readiness & Risk Management
|
- Change impact assessments that identify what is changing, who is affected, and what support is needed (including equality and inclusion considerations).
- A change risk and issue log for people impacts, with clear actions, owners and review dates.
- Readiness and capacity updates that help the programme make informed decisions about sequencing, pace and go‑live dates.

Communication & Engagement
|
- A communication plan that is clear, timely and accessible (including channels, messages, owners and milestones).
- Simple, plain English messages that explain the why, what, when and what it means for people in practice.
- Listening and feedback routes (e.g., sessions, surveys, drop‑ins) with themes reported back and actions agreed.

Capability Building & Adoption
|
- A capability needs assessment and learning plan for the change (skills, knowledge, confidence and support needed by different roles).
- Manager and team toolkits that help people lead and navigate change day to day (briefings, FAQs, templates and guidance).
- Adoption actions agreed with service and programme leads (reinforcement, local champions, recognition and ‘what good looks like’).

Measurement & Evaluation
|
- Success measures for the people side of change are agreed and tracked (e.g., awareness, readiness, adoption, confidence and key behaviours).
- Regular reporting that shows what is working, what is not, and what we will do next—using both data and feedback.
- Lessons learned are captured and shared so the Trust’s change approach improves across the transformation programme.
